Annual Health Check at Your GP Practice
If you have a long-term condition such as diabetes, asthma, COPD, or high blood pressure or heart failure ,you are invited to attend an annual LTC review β usually around your birthday month π
π©Ί What Happens at Your Review?
Your appointment is usually with a Practice Nurse or Healthcare Assistant (HCA) and may include:
βοΈ Blood tests , Blood pressure, Heart rate
βοΈ Weight check
βοΈ ECG (if required)
βοΈ Discussion of symptoms for your ongoing conditions.
βοΈ Medication discussion
Youβre welcome to bring a family member, friend, or carer for support.
π Why Is This Important?
LTC reviews help us to:
β’ Monitor your condition safely
β’ Review medications
β’ Provide education & lifestyle advice (diet, exercise, smoking)
β’ Spot problems early
β’ Reduce the risk of complications or hospital admissions
Your named GP will review your results, update medications, and renew repeat prescriptions if appropriate.
NHS app:
We are aware that a lot of our patients order their repeat prescriptions on the NHS app which is quick and convenient. You may find that you are unable to order your medications on the NHS App until you have had your review.

If you have any concerns youβd like to discuss with your GP, please tell the Nurse or HCA during your review, and your GP will contact you directly.

β‘οΈ Patients are responsible for booking their LTC review appointment.
β What If I Miss My Appointment?
β’ Please reschedule as soon as possible
β’ Prescriptions may be delayed if reviews are overdue
β’ Up to three reminders may be sent before a repeat prescription is paused
